When a potential Surrogate is about to embark on a surrogacy journey, a comprehensive medical record review is required as part of the qualifications for a suitable candidate. In many cases this is the first step in Fertility Specialists’ evaluation of a candidate. The NSRR Medical Record Review consists of collecting and vetting all available prenatal and delivery records from previous pregnancies, as well as written medical clearance from a candidate’s physician. NSRR will draft a summary of pregnancies, pregnancy history, and medical/operative history. Additionally, we also require a established clearance form to be completed by the physician who cares for her stating she does or does not not meet the criteria to continue as a Surrogate.
Once our report is generated the product is yours to utilize. The organized summary and reporting can be sent directly to coordinators at the Fertility Center or Surrogacy Agency to contact the Intended Parents and Surrogate next steps.
We also complete medical records reviews for egg & sperm donors, a service that is becoming increasingly in-demand.
Our all-inclusive medical records review and report is a flat-fee of $500. Turn-around time for summaries is within 3-5 business days. We do offer a rush service; add $200 for a 24-48 hour fast track report.
